SOCIAL WORKERS – Grade 2 or 3

Who are we?

Robinvale District Health Service (RDHS) provides flexible and coordinated health care services for communities in Victoria’s accessible Outback – we work in northwest Victoria, and parts of New South Wales. We provide aged care, sub-acute and allied health services on a primary care model that meets our communities’ health needs across the lifespan. We work holistically and we empower individuals, families and communities to take charge of their own health.

We are seeking a Social Worker – Grade 2 or Grade 3 to work in a full-time basis (80hrs per fortnight, including ADO’s – ongoing)

ABOUT THE ROLE
GRADE 2:

The Grade 2 Social Worker is an experienced clinician who works with individuals, families, groups and communities to assist with, and improve well-being through casework, counselling, advocacy and community development.

The role involves working with acute inpatients, outpatients and aged care residents and includes psycho-social assessment, discharge planning, education and referral.

The Grade 2 Practitioner may also have responsibility for the supervision of students.

GRADE 3: Team Leader

The RDHS Social Work Team Leader works as part of the Primary and Community Services team, although has a priority focus on Social Work service delivery and Social Work team support.

The Social Work Team Leader possesses a higher level of clinical competence and experience, however is still involved in direct client care, both onsite and offsite, including Outreach services and health promotion activities to address the health needs of the community.

The caseload is spread across a variety of locations including: RDHS Health and Wellbeing centre, acute hospital, aged care facilities, home-based, Outreach clinics and other locations that can arise.

The Social Work Team leader will be expected to work with students and Allied Health Assistants.

Job Requirements:

GRADE 2:

  • Tertiary qualification in Social Work and eligibility for AASW membership.
  • At least two years’ experience working the Social Work field.
  • Proficient clinical skills in psychosocial assessment, risk assessment, complex care planning, social work interventions and discharge planning.
  • Demonstrated understanding of theoretical frameworks which inform Social Work practice.
  • Effective communication and organisational skills.

GRADE 3:

  • Tertiary qualification in Social Work and eligibility for AASW membership.
  • Several years’ experience working in the Social Work profession.
  • Qualifications and/or experience in leadership and management.
  • Advanced clinical skills in psychosocial assessment, risk assessment, complex care planning, social work interventions and discharge planning.
  • Demonstrated understanding of theoretical frameworks which inform Social Work practice.
  • Experience and understanding of issues specific to provision of services in rural and remote areas.
  • Effective communication and organisational skills.
